Important information for HMO members

If your insurance card says HMO and lists another provider’s name, we cannot see you until your primary care provider is changed to Dr. Robert Caiati.

To make the change, call the member services number on the back of your insurance card. Ask your plan to update your primary care provider to Dr. Robert Caiati at Babylon Primary Care. Then call our office to schedule your visit.
